• Full legal name at the time of exit or graduation from Churchill
• Birth date
• Last year of attendance or graduation date
• Address where you would like the transcript sent

Unofficial transcripts can be emailed directly to you. Official transcripts are signed, stamped, and sealed in an envelope to be mailed. Please allow 24 hours for processing.
