Locker Request
Student Locker Rules
Lockers belong to the school district and are assigned for the convenience of students. At Churchill, lockers are only assigned to students who request them.
- No locker sharing. One student per locker. DO NOT share your locker combination with anyone or allow anyone else to store items in your locker.
- Do not remove the school-issued lock. Locks must stay on the locker and must be locked at all times. Only unlock the locker to deposit or retrieve your items. Do not replace the school-issued lock with a personal lock or other lock that does not belong to the school.
- Take care of your locker. Keep it clean. Lockers are not to be used for the storage of illegal or dangerous items, items prohibited by school rules, or evidence of an illegal act or a violation of a school rule.
- School staff may inspect your locker. Inspections of individual lockers may be made when there is reasonable suspicion to believe that illegal or dangerous items or items that are evidence of a violation of the law or school rules are contained in them. (See the 4J Student Rights and Responsibilities Handbook.)
- Lockers must be cleaned out by the last day of the school year, or by the student's last day if the student leaves Churchill before the end of the school year. All items remaining in lockers will be discarded or donated immediately after the last day of school.
This request form is only for general school lockers. Gym lockers are assigned separately through the Physical Education (PE) teacher or the Athletic Department for students enrolled in PE classes or participating in sports.
