Step 1

• Complete a Behavior Referral Form (available in the front office). 
  • Major Incident Behavior Referral (pink) (PDF)
  • Minor Incident Referral (yellow) (PDF)

Step 2

Submit the completed form to:

• 9^th and 11^th Grades: Beth Kruziki
• 10^th and 12^th Grades: Justin Huntley

Follow Up

• An administrator will make a connection with the student(s) and determine the appropriate next steps.
