Registration and Eligibility
Registration is currently closed. Check back in August for updated registration information.
Eligibility Requirements
Student-athletes must…
- complete the online registration; you will need the following information:
- student, parent/guardian, and emergency contact information
- student’s medical insurance policy information (student accident insurance is available at www.myers-stevens.com/enrollment-page)
- student’s medications and medical history
- submit the student’s most recent OSAA physical examination form (español) (valid for 2 years from the date of the medical appointment, and must be valid through the end of the sports season)
- submit any eligibility forms that apply to the student-athlete:
- pass the Academic Requirements for Athletes
- pay fees
- Pay in the Finance Office with cash, check, or card.

Seasonal Sports Costs
Cut Sports: All fees are paid up front. Checks/money will held for cut sports until teams are finalized. Checks/money will be returned to students cut from the team.

*Sports Reduced Fee Qualifications: In order to qualify for the reduced fee you must provide the free and reduced determination letter.
**ASB Fees: All students must have an ASB membership to participate in athletics at Churchill. Students will only be required to pay the ASB fee once per school year.
***Registration Fee: Student-athletes will only be required to pay the registration fee once per school year.
Community-Sponsored Activities
School- and Community-Sponsored Activities Guidelines
The Eugene School District 4J encourages students to be involved in extracurricular and co-curricular activities. In addition to school-sponsored activities (school clubs), Community-Sponsored activities allow students to broaden their educational experience in the areas of physical fitness, social development, and skill enhancement.
Non-school-sponsored activities are independent of and receive no funding, services, or supervision from the Eugene School District/4J. Community Sponsored activities must submit this application and release annually to the building principal at the school in which they wish to be recognized.
